News in Virginia

Outbreak of Extensively Drug-resistant Pseudomonas aeruginosa Associated with Artificial Tears. On February 01, 2023 , CDC issued a Health Alert Network Advisory about infections with an extensively drug-resistant strain of Verona Integron-mediated Metallo-β-lactamase (VIM) and Guiana-Extended Spectrum-β-Lactamase (GES)-producing carbapenem-resistant Pseudomonas aeruginosa (VIM-GES-CRPA) in 12 states.  CDC is collaborating with the Food and Drug Administration (FDA) and state and local health departments to investigate the outbreak. The investigation to date has identified artificial tears as a common exposure for many patients. CDC and FDA recommend clinicians and patients stop using EzriCare or Delsam Pharma’s Artificial Tears products pending additional guidance from CDC and FDA. 

 

As of March 01, 2023, Virginia has not identified cases linked to this outbreak. Clinical laboratories and healthcare professional should continue to follow the Virginia Reportable Disease list which states: 

  • Clinical laboratories should forward all carbapenem-resistant Pseudomonas aeruginosa isolates to the Division of Consolidated Laboratory Services (DCLS) for further public health testing 
  • Healthcare providers should report all carbapenemase-producing organisms (CPO), infection or colonization, to your local health department (LHD) 

Spotlight on Regional Infection Preventionists, Northern Region. Northern Region Infection Preventionist (IP) Scott Baumgartner has been with VDH since October 2021. During his time as a Regional IP, he has been grateful for the opportunity to partner with his local health districts and with healthcare facilities, including long-term care facilities, in the region. Scott has done multiple onsite infection prevention and control (IPC) visits which have helped facilities grow their IPC programs and improve their IPC practices 

Providing education has been a key part of Scott’s work as an IP. He has led trainings on topics such as Enhanced Barrier Precautions, N95 fit-testing (train-the-trainer), and IPC lessons learned during the COVID-19 pandemic. Scotts largest educational endeavor was creating a comprehensive eight-hour training course for all new hires at a skilled nursing facility in Northern Virginia, covering all aspects of IPC. Current staff members receive a one-hour abbreviated version of the course. This education activity introduces new IPC topics and reinforces the facility’s IPC policies. The training continues to be offered by facility staff, contributing to a more robust education plan to help protect the facility’s residents and staff.  

 

Reportable Disease Regulations Updates. Amendments to the Virginia Regulations for Disease Reporting and Control (amending 12VAC5-90-80 and 12VAC5-90-90) were effective January 18, 2023. Updates to the Disease Reporting and Control Regulations webpage and Reportable Disease List posters are planned and will be shared when available via VDH clinician letter.  The changes separate COVID-19 from the category “Coronavirus, severe (e.g., SARS-CoV, MERS-CoV)” on the reportable disease list and remove the requirement for COVID-19 to be rapidly reportable.  The amendments also add to the reporting requirements: 

  1. For physicians and directors of medical care facilities to report to the health department not only when a person who is infected with or who is suspected of having COVID-19 is treated or examined, but also if the person is hospitalized or admitted into an intensive care unit;  
  2. Clarity that the category "laboratory directors" includes pharmacies that hold Clinical Laboratory Improvement Amendments Certificates of Waiver so that pharmacies testing for COVID-19 are required to report to VDH;  
  3. Specificity that only positive SARS-CoV-2 test results shall be reported;  
  4. That the patient telephone number and ethnicity be included in the list of fields that are reported by physicians, laboratory directors, and directors of medical care facilities; and  
  5. That "coronavirus, severe" is added to the list of infectious diseases that shall be reported to persons practicing funeral services.

Respiratory Diseases. 

 

Influenza: February was American Heart Health Month; people with heart disease and those who have suffered from a stroke should get a flu vaccine every flu season to protect against flu and its potentially serious complications. Among adults hospitalized with the flu, CDC reports that heart disease was one of the most common chronic conditions and that serious heart complications can occur in one out of every eight patients.  Furthermore, a CDC study found the risk of having a heart attack to be six times higher within a week of confirmed flu infection.  

 

As of February 10, 2023, CDC reported 106 pediatric flu-related deaths for 2022-2023, the highest number of pediatric flu deaths since the start of the COVID-19 pandemic. Flu vaccination uptake in children is lower by about six percentage points that it was prior to the pandemic. Although flu activity has returned to low levels this season, the CDC continues to recommend flu vaccination. Flu vaccines are reviewed each year and updated as needed to protect against the latest flu viruses. A flu vaccine protects against the four flu viruses that research indicates will be the most common during the upcoming season. COVID-19: In January, the US Food and Drug Administration announced that Evusheld is not currently authorized for emergency use because it’s likely ineffective against more than 90% of SARS-CoV-2 variants now circulating in the US. Evusheld had been used as COVID-19 preexposure prophylaxis among certain patients who are immunocompromised. In light of this change, CDC released new recommendations for people who are moderately or severely immunocompromised 

 

Based on current COVID-19 trends, the U.S. Department of Health and Human Services (HHS) is planning for the federal Public Health Emergency (PHE) for COVID-19, declared under Section 319 of the Public Health Service (PHS) Act, to expire at the end of the day on May 11, 2023. 

 

Tuberculosis. 

World TB Day: The theme of this year’s World TB Day is “Yes! We Can End TB.” The VDH TB Program will recognize this important day by hosting a webinar at 2:00 PM ET on March 24th with programmatic and epidemiologic updates and a lecture on TB and mental health presented by Dr. Annika Sweetland, Chair and Co-Founder of the Union Working Group on TB and Mental Health. 2022 TB Data Now Available to Inform Facility TB Risk Assessments: 2022 TB counts and rates by city/county are available for Virginia. You can access the updated information directly here, or on the VDH TB Program's Data and Reports page under "Local Data". For low morbidity cities/counties, it may be helpful to look at the regional rate, or at the rates of surrounding areas, when performing your annual risk assessment. Please reach out to tuberculosis@vdh.virginia.gov with additional questions.

NHSN Training Updates. Mark Your Calendar! CDC’s National Healthcare Safety Network (NHSN) will hold the Virtual 2023 Annual NHSN Training: Patient Safety, Outpatient Procedure, and Neonatal Component Healthcare Surveillance and Analytics on March 21 – 23, 2023. This training is intended for users of the Patient Safety Component, Outpatient Procedure Component, and Neonatal Component in NHSN. The virtual training event will feature live presentations, pre-recorded training videos for self-paced viewing, an introduction of new NHSN Measures, and opportunities for Q&A. More information, including registration and an agenda, will be provided soon on the CDC website.

American Health Care Association: Join the new LTC National Infection Prevention Forum. The American Health Care Association (AHCA) has partnered with the Association for Professionals in Infection Control and Epidemiology (APIC) to set up a national forum for infection preventionists and infection prevention champions working in long- term care (LTC). The forum provides a platform to rapidly disseminate updates and resources about infections in LTC as they become available from CDC, Centers for Medicare and Medicaid Services (CMS), or Occupational Safety and Health Administration (OHSA). The goal is to foster collaboration and sharing of experiences between IPs and other infection control champions in LTC. Participants will be encouraged to answer questions and share resources and tools. If you are interested in participating, please complete this three-question survey to provide your name and email. 

 

This is open to all LTC professionals focusing on infection prevention regardless of their infection prevention certification status or membership in either APIC or AHCA. Research Corner

Investigation of a Candida auris Outbreak in a Skilled Nursing Facility – Virginia, United States, October 2020 – June 2021. In December 2022, VDH staff from the Alexandria Health Department and the HAI/AR Program published their report on a C. auris outbreak investigation at a Virginia ventilator-capable skilled nursing facility (vSNF). Candida auris is an emerging multidrug-resistant organism (MDRO). During October 2020–June 2021, the facility identified 28 cases among residents in the ventilator unit. They detected multiple infection and prevention control challenges, highlighting the importance of strengthening MDRO prevention efforts at vSNFs. Vital Signs: Health Disparities in Hemodialysis-Associated Staphylococcus aureus Bloodstream Infections, 2017-2020. Health disparities are inequitable and are directly related to the history and current unequal distribution of social, political economic, and environmental resources. Racial and ethnic minorities are disproportionately affected by end-stage kidney disease (ESKD). A study was conducted to discover how ESKD patients on dialysis are at increased risk of Staphylococcus aureus bloodstream infections, while also taking a closer look at the racial, ethnic, and socioeconomic disparities associated with this outcome.
