CLABSI Event

Join a statewide conversation between the Virginia Department of Health (VDH), hospitals, and the Virginia Hospital & Healthcare Association (VHHA) about the recent state of healthcare-associated infections in Virginia. During the webinar, we will: 

  • Present annual HAI data for acute care hospitals before and during the pandemic, including the impact, efforts made, and future plans 
  • Focus on central line-associated bloodstream infection (CLABSI) standardized infection ratios and challenges faced 
  • Share lessons learned from the field 
  • Discuss how VDH and VHHA can support hospitals in their prevention efforts 

 

Prior to the webinar, an infection preventionist from each hospital will be requested to complete a short survey regarding their experience with CLABSI in their facility.

News in Virginia

Spotlight on Regional Infection Preventionists, Northwest Region. 

The Northwest IP team, Holly Spindle and Kayleigh Rehkopf, have been grateful for the opportunity to partner with long-term care facilities to do onsite infection prevention and control (IPC) visits and see the facilities' great IPC programs in practice, including IPC auditing, and fit testing for N95 respirator. They also provide education fairs that address topics like hand hygiene, personal protective equipment (PPE) donning and doffing, and environmental cleaning and disinfection. 

 

At a recent staff education fair at an independent living facility in the Blue Ridge Health District, the team experienced firsthand the importance of resident education. Facility residents play a key role in infection prevention. Part of the job of an IP is to ensure that residents know what they can do to proactively reduce the risk of spreading infection in the facility. A resident who observed the education fair, approached the Northwest IPs asking questions about the luminometer and how it worked. The IPs provided education on surface contamination and demonstrated using ATP swabs to evaluate how well a surface is cleaned. The resident was highly interested and showed the IPs what she learned by cleaning her walker with a disinfectant wipe, including following the contact time, and said that she would begin cleaning her walker every day! 

 

As a result of the resident interaction, the Northwest IP team has planned collaborations with the Virginia Department of Social Services to develop resident education for independent and assisted living facilities. The education will ensure residents have the knowledge to identify risks and understand their role in proactively reducing risks associated with the of spread of infections.  Additionally, since their educational interaction with the resident in Blue Ridge, two other facilities have also requested resident education. 

Influenza.

COVID-19 Updates. In late December, the White House published the Winter Playbook for Nursing Homes and Other Long-Term Care Facilities to Manage COVID-19 and Protect Residents, Staff, and Visitors. This resource defines the three priorities that long-term care facility (LTCF) leaders should focus on this winter to reduce the impact of COVID-19 in LTCFs and offers strategies to accomplish those goals: 

  • Help residents and staff access updated COVID-19 vaccines 
  • Ensure that residents and staff are testing if they are symptomatic, and know how to access COVID-19 treatment options (such as oral antiviral pills) if they test positive 
  • Improve indoor air quality across facilities 

Tuberculosis. 

  • The CDC Division of Tuberculosis Elimination has released their comprehensive report on TB in the U.S. for the year 2021. This report contains information on TB cases from 1993 to 2021 and features a series of reports on subjects such as national, state, and local data, risk factors, demographics, TB treatment and case outcomes, drug-resistant TB, genotype clusters, and estimates of recent transmission.

CDC is Tracking Possible Rise in Group A Strep. The CDC is looking into a possible increase in invasive group A strep (iGAS) infections among children in the U.S. Infections of iGAS include necrotizing fasciitis and streptococcal toxic shock syndrome. NHSN

Transition of COVID-19 hospital data reporting to NHSN. The National Healthcare Safety Network (NHSN) assumed responsibility for collection of COVID-19 hospital data on Thursday, December 15, 2022.  COVID-19 hospital data should be reported to NHSN as of December 15, 2022. Hospitals should continue to report COVID-19 data while NHSN is working on addressing application issues.

CMS Quarterly Data Reporting Deadline. The deadline to enter 2022Q3 data into NHSN for the CMS Quality Reporting Programs for participating acute care hospitals, long-term acute care facilities, inpatient rehabilitation facilities, and cancer hospitals is March 15, 2023. To ensure your data have been correctly entered into NHSN, please verify that: 1) your monthly reporting plans are complete, 2) you have entered appropriate summary and event data or checked the appropriate no events boxes, and 3) you have cleared all alerts from your NHSN facility homepage. Hospitals that have conferred rights to VDH will be receiving a quality assurance report in the second week of March, so please be sure to check your email and acknowledge receipt and review. Please remember to update the VDH HAI/AR Program with any IP contact changes. For additional guidance on ensuring your data are accurately sent to CMS for Quality Reporting purposes, please visit the NHSN website and navigate to the appropriate section(s) for your facility type.

Training & Education

Enhanced Barrier Precautions. Questions about implementing Enhanced Barrier Precautions (EBP) in your nursing home? Check out the latest updates to the VDH frequently asked questions about EBP. If you have additional questions specific to your facility, reach out to hai@vdh.virginia.gov for guidance and we can connect you to an infection prevention expert in your area.  

 

CPO Alerts in the EDCC Program. Starting on December 7th, multidrug-resistant alerts for Carbapenemase Producing Organisms (CPOs) appeared in the electronic medical record of facilities participating in the EDCC (Emergency Department Care and Coordination) Program. Like the C. auris alerts that began September 1st, these alerts will appear indefinitely for patients/residents with previous CPO clinical or colonization diagnosis reported to VDH. To prepare, facilities should:  

  • Update facility-specific CPO guidance and align guidance with CDC recommendations  
  • Create processes to ensure patients/residents with a CPO flag will be promptly placed in the appropriate transmission-based precautions  
  • Notify and educate appropriate staff about the new CPO flag  
  • Continue to communicate CPO status directly with other transferring facilities due to any potential lag in data transfer

Project Firstline.  

  • Join a new LTC IPC Forum: The American Health Care Association (AHCA) has partnered with the Association for Professionals in Infection Control and Epidemiology (APIC) with funding from CDC Project Firstline to set up a national forum for IPs and infection prevention champions working in long-term care (LTC) to share resources and get questions answered. This is open to all LTC professionals focusing on infection prevention regardless of certification status or membership in either APIC or AHCA. If you are interested in participating, please complete this three-question survey to provide your name and email.  
  • Project Firstline’s NEW Spanish Resources: Check out the new infection control educational materials and resources that are based on the risk recognition content and were created for Spanish-speaking frontline healthcare workers. The Spanish section of the Project Firstline website has also been reorganized to allow for easier navigation and accessibility of resources. 
  • New Antimicrobial Fact Sheet: Project Firstline released a new fact sheet to help educate frontline healthcare workers on the important role they play in fighting antimicrobial resistance with infection control.  

 

How to Develop a Respiratory Protection Program. There is a new resource to help your facility develop a respiratory protection program (RPP). The goal of an RPP is to prevent the spread of small aerosol germs or particles that can cause disease. Most places that provide healthcare services are required to have an RPP. You should develop your RPP to meet the needs of your facility. Looking to build your knowledge of infection prevention and control? If so the Virginia Infection Prevention Training Center is pleased to announce a FREE on-demand Foundations Course in Infection Prevention & Control, available starting January 23, 2023.

Research Corner

CDC Core Infection Prevention and Control Practices for Safe Healthcare Delivery in All Settings. In November, CDC updated the core set of infection prevention and control practices that are required in all healthcare settings, regardless of the type of healthcare provided. Originally published in 2014, changes to the document include: 

  • Adding an injection and medication safety practice regarding where to safely prepare medications 
  • Adding a minimizing potential exposure practice regarding developing and implementing systems for early detection and management of potentially infectious persons at initial points of patient encounter in outpatient settings and at the time of admission to hospitals and long-term care facilities 
  • Adding consideration for broader use of source control during periods of higher levels of community respiratory virus transmission 

 

2021 National and State HAI Progress Report. The CDC 2021 National and State Healthcare-Associated Infections (HAI) Progress Report provides a summary of select HAIs across four healthcare settings: acute care hospitals, critical access hospitals, inpatient rehabilitation facilities, and long-term acute care hospitals. This report, along with the detailed technical tables, provides national- and state-level data about HAI incidence during 2021.
