We are reaching out to announce that the COVID Case Manager (CCM) program will be ending, effective Friday, March 22, 2024.
The CCM team provided long-term care (LTC) facilities throughout Minnesota with COVID-19 guidance and resources throughout the pandemic. This team supported countless LTC communities during an unprecedented time. We appreciate the support that they have provided for LTC, and we are here to continue to support facilities in responding to COVID-19 and other respiratory illnesses as needs arise.
