Information included in your registration is subject to verification, validation and periodic
updates through sharing between local, state, tribal, or federal governmental entities
(collectively referred to hereafter as “Government”). By accepting this disclosure, you
are authorizing the State of Oklahoma, Department of Veterans Affairs to access and
review your records with the United States Department of Veterans Affairs for the purposes
stated above.
Failure to provide information required for registration or in connection with verification
of eligibility may result in a denial letter and omission from the Veterans Registry until
the required information or documentation is provided and qualifying information is substantiated.
Information Sharing:
Individual information shall be available to Government entities, and will not be shared
with private entities
without the express written permission of the veteran or their authorized agent.
You may be contacted by a Government representative. Descriptive
statistical information on the Oklahoma veteran population and any subset thereof may
be shared with governmental or private entities.
ODVA may wish to send you regular updates or communications such as newsletters

A future capability of the Oklahoma Veterans Registry may permit sharing of veteran information after the veteran’s death for historical reporting purposes, including but not limited to genealogical and family history research. Terms of Participation in Registry:
The purpose of the Veterans Registry is to enhance the accuracy of accounting for
Oklahoma’s past and present veterans, develop a better understanding and meet the
needs of Oklahoma’s veterans, facilitate outreach programs, develop a contact network
through which optional communication on topics of interest to veterans, including but
not limited to availability or updates of state or federal benefits, enhance the accuracy
and ease of identification of veterans through indicators on state-issued identification
cards and drivers licenses and create and offer a historical record to preserve the
military service histories for Oklahoma veterans for genealogical and general historical
records purposes. Data gathered may be utilized for one or more of these purposes.
Participation in the Oklahoma Veterans Registry is Voluntary. Eligibility for benefits offered by the State of Oklahoma may be negatively impacted by your election not to participate. Eligibility for federal benefits and services are not subject to participation in the Oklahoma Veterans Registry.
You may change any election contained in this form at any time by submitting your request in writing to the Oklahoma Department of Veterans Affairs, Attn: Veterans Registry, 2132 NE 36th Street, Oklahoma City, OK 73111. You have the right to decline participation in the Oklahoma Veterans Registry. You may receive a copy of this form or your record within the registry upon receipt of your written request.
The Oklahoma Veterans Registry consists only of the information provided on the form. Medical records are not included in the Oklahoma Veterans Registry, and participation does not authorize the release of your medical records. A copy of this form shall be deemed effective in the same manner as an original.
Additionally, the Oklahoma Department of Veterans’ Affairs may verify your eligibility for any benefits offered through the State of Oklahoma for which you qualify, including but not limited to: military license plates, sales tax exemptions, reduced rate vehicle tags, and/or veterans’ identifier on your state issued ID.
