Disability services provider public health grants

A grant program was recently created to prevent the spread of COVID-19 to people with disabilities and staff who support them. We will do this by:

  • · Continuing or increasing use of individualized day or employment services
  • · Reducing use of congregate and sheltered workshop settings.
  • Who is eligible for this grant?

    Providers of services below are eligible for a grant:

    1. Brain Injury (BI) Waiver:

  • · Employment exploration, employment development and employment support services
  • · Prevocational services
  • · Structured day services
  • 2. Community Access for Disability Inclusion (CADI) Waiver:

  • · Employment exploration, employment development and employment support services
  • · Prevocational services
  • 3. Community Alternative Care (CAC) Waiver:

  • · Employment exploration, employment development and employment support services
  • 4. Developmental Disabilities (DD) Waiver:

  • · Employment exploration, employment development and employment support services
  • · Day training and habilitation services
  • How can you apply?

    Submit an application using the Disability Services Online Grant System Logon Page by 11:59 p.m. on Nov. 16, 2020. We will not accept applications received after the deadline.

    Providers must agree to several conditions to get this grant.

    How much money will each grant award be?

    The maximum initial grant is $200,000. If funds are available after all eligible providers have received a grant, we may award additional funds to providers who have already received the $200,000 initial grant.

    If we receive requests for more money than is in the fund, priority will be given to applications that demonstrate the most need or the most robust plan to ensure people have opportunities to participate in day or employment services that are not provided in a facility or sheltered or work crew setting. Our determination of a provider’s grant amount is final and not subject to appeal.
