August 2022

Welcome to our August 2022 diabetes communications update. We aim to keep you informed on the diabetes work of Strategic Clinical Networks (SCN) and developments that may impact people living with or at risk of developing diabetes in Greater Manchester (GM) and those providing their care.

We want to make sure that GP practices, primary care networks, trusts and CCGs are kept up to date, so please do share these with colleagues and contacts. Please also let us know if you will like us to share important news or information with other stakeholders in Greater Manchester by emailing winifred.bajulaiye1@nhs.net.

Healthier You - Diabetes Prevention Programme

Luis

The Heathier You National Diabetes Prevention Programme would like to share the latest good news on the programme.

Over 28,000 people have been referred to in Greater Manchester over the past 3 years.

Participants who complete Healthier You have achieved an average weight loss of 4kg. Outcomes have been particularly good for people who have accessed the digital programme with completers achieving an average weight loss of 4.9kg.

Research from the University of Manchester has found that people also reduce their chances of developing diabetes by more than a third (37%).

We have many success stories from our Healthier You participants. Luis who attended healthier You reflected that “When we go to the course, we discuss things that motivate you. The changes we’re making are really sustainable. I really feel better! I really look forward to the programme – it’s local, the coach is very friendly and helpful, and the materials we are given are very easy to use and understand.”

NHS Low Calorie Diet Programme

Krista Williams Senior Quality Improvement Manager (Diabetes) is now leading on coordinating the Low-Calorie Diet programme for the SCN in Greater Manchester. Krista would like to offer direct tailored support to primary care practices through Xyla’s engagement team who will help practices identify, contact and recruit patients to the Low-Calorie Diet Programme.

Xyla’s LCD engagement team can support practices to:

  • Run an EMIS search to identify patients eligible for the LCD programme
  • Send texts or letters to patients inviting them to join a patient information session held in the practice or suitable close venue where patients will learn more about the programme and what it entails
  • Refer those patients that are interested to the programme, providing hands on support to complete the referral and medication deprescribing.

Practices will be eligible for an £11.50 payment for every patient referred if they meet the criteria of the Weight Management Enhanced Service.

For the opportunity to discuss in more detail about the offer and what it involves please contact kristawilliams@nhs.net
