Diabetes Update - December 2022

Welcome to our latest Diabetes bulletin. We aim to keep you informed on the diabetes work of Strategic Clinical Networks (SCN) and developments that may impact people living with or at risk of developing diabetes in Greater Manchester (GM) and those providing their care.  We want to make sure that GP practices, primary care networks, trusts and localities are kept up to date, so please do share these with colleagues and contacts.  Please also let us know if you will like us to share important news or information with other stakeholders in GM by emailing winifred.bajulaiye1@nhs.net.

New culturally appropriate diabetes education planned for GM

Structured Diabetes Education (SDE) is available in digital form for people living in GM. Click here to find out more.

The GM Diabetes Board’s Structured Education Steering Group are planning to develop and test adapted education courses designed to be more appropriate for some of the diverse communities living in GM.  This will include SDE programmes for people from:

  • The Deaf Community
  • Black African and Caribbean communities
  • South Asian communities

Increasing the number of patients accessing their diabetes data

NHS England are accelerating citizen access to GP data from 1st November 2022. An increasing number of people living with diabetes in GM are accessing their own diabetes GP data already through Diabetes My Way (DMW).

  • Almost 7,500 people in GM have registered to use the DMW service.
  • 143 GP practices have signed up activated the EMIS bulk data handling service.
  • Over 335,000 website visitors have completed more than 1,411,000 web page views.
  • MyWay Digital Health can manage the patient communication and registration service for any practice that requests it.

Driving Change & Tackling Inequalities in Manchester

GM Diabetes Network Clinical Lead, Naresh Kanumilli (pictured) and SCN diabetes programme lead Ewan Jones presented the GM Diabetes Strategy and led workshop discussions at an collaboration event in convened by Boehringer Ingelheim.

The purpose of the event was to consider how to tackle health inequalities experienced in diabetes care. It included participants from localities around GM and participants concluded there is a need for the GM the system to focus on some key goals, including:

  • Deliver accessible structured diabetes education for all people.
  • Support the development of expert patient groups in community practice to provide support and education.
  • Improve patient engagement and the patient journey using administrative care coordinator roles.
  • Develop a one-stop-shop approach for patients to receive diabetes care processes.
  • Review long-term conditions in integrated fashion for patients via combined clinics.
  • Take a co-ordinated approach to diabetes prevention from pre-conception to adolescence including education provided at school.

New transition funding for children and young adults with diabetes in Stockport

Following on from the work of the Children Young Persons national ‘Improving equity of access to treatment technology’ working group, funding has been made available to each NHS region for targeted interventions to improve equity of access to diabetes treatment technology.


A key area of focus is improving care for those transitioning from paediatric to adult care and addressing the poorer outcomes for children and young adults with diabetes at transition age. Funding for pilots over the next three years has been agreed and 15 sites have been selected:

  • Airedale NHS FT and Bradford Teaching Hospitals NHS FT
  • Barts Health
  • Dorset ICS
  • Ipswich and East Suffolk
  • Kings Health Partners
  • Liverpool University Hospital and Alder Hey children’s Hospital
  • Newcastle upon Tyne Hospitals
  • Norfolk & Norwich University Hospital
  • Oxford University Hospitals
  • Sherwood Forest, Kings Mill and Newark Hospital
  • Southampton General Hospital/ Royal South Hants/Southern Health
  • Southport and Ormskirk Hospital
  • Stockport NHS FT
  • Torbay and South Devon
  • University Hospital of Derby and Burton

The results from these pilots will provide valuable insights and help bridge the evidence-gap that exists with regards to clinically effective transition and young adult care models.  Further information on the Stockport Pilot is available from Sarka.grayson@nhs.net.

 


Low Calorie Diet Programme

On World Diabetes Day, Derek from Stockport kindly shared his experience of taking part in the NHS Low Calorie Diet programme (LCD). Click here to read Derek's story. Your patients can access this programme too:

  • LCD is available to people in GM who are overweight and living with type 2 diabetes. It provides treatment for people to improve their diabetes control, reduce diabetes-related medication and in some cases, put their type 2 diabetes into remission. The average weight loss to date is 13.4kg and findings show that people manage to keep it off.

Healthier You NHS Diabetes Prevention Programme

In the New Year the Diabetes network will be leading a social media marketing campaign for the Healthier You programme to encourage people to engage with the service. A number of posts that can be used on social media will be shared with local communication teams in GM. The campaign will focus on reminding people of the risks of developing type 2 diabetes and how to find out their risk using the Diabetes UK Know Your Risk tool. NICE update

NICE have published the review questions for ‘NICE guideline on Diabetes (type 1 and type 2) in children and young people: diagnosis and management - medicines for type 2 diabetes (update)’.

 The review questions are available on the project documents web page. This information is published for transparency, and to enable stakeholders to prepare for consultation on the draft guideline.

  • The consultation on the draft guideline and its supporting evidence is due to start on 25-Jan-2023 and it will run for 4 weeks, closing on 22-Feb-2023.

New digital standards planned for sharing diabetes data

The Professional Record Standards Body (PRSB) was commissioned by NHS England to produce standards for sharing diabetes information between people and professionals across all care settings, including self-management data from digital apps and medical devices. Find out more here.  The information record standard has been submitted to the Data Alliance Partnership Board to gain Information Standard Notice Status (ISN). Future work & forward planning

The Diabetes Board and Programme Team are already engaged in or are planning work on a number of issues, including:

  • Supporting implementation of 2022 updates to NICE guidance NG17, NG18 and NG28 on the use of CGM devices.
  • Producing a ‘gold standard’ structured diabetes education referral & management pathway.
  • Supporting deployment of a cardio-renal-metabolic pathway.
  • Embedding key services like the Manchester Amputation reduction Service and Diabetes My Way in the GM diabetes care system.
