Compliant DL–ID Card - Acceptable Documents List

Once completed, print and/or email the checklist to help you acquire all needed documentation before going to a Deputy Registrar Agency to obtain your driver license or identification card.

To prove identity, you must show proof of the following five elements

1-Full Legal Name*; 2-Date of Birth; 3-Proof of U.S. Legal Presence; 4-Social Security Number; 5-Proof of Address

To prove an element, the element must be contained on the document. One document from this list may be presented for a duplicate or renewal issuance (does not apply to Non U.S. citizens).

TWO documents from different sources proving Ohio Street Address that are listed on the Acceptable Document List are required for proof of Ohio residency.

NOTE: Customers are encouraged to use this checklist to prepare in advance of visiting a Deputy Registrar agency. Use of this checklist does not guarantee issuance of a DL-ID. Issuance will be based on the validity and acceptance of actual documents presented at the time of the transaction.

NOTE: STATE ISSUED DRIVER LICENSE - Issued by any state, territory, or possession of the United States, with photograph, valid and unexpired, is acceptable for waiving driver exam testing requirements.
