Consumers

Anyone can notify the Drug Price Transparency Program of an increase in the cost of prescription drugs. Consumer notifications are published on the Data webpage. The information may also be used in the annual report to the Oregon Legislature.

We encourage people to provide notification of an increase in the cost of their prescription drugs. Upon request, we will provide options in an alternate format for people with disabilities or in another language. Fill out the online form or use a fillable form below that can be downloaded in different languages.

If you have questions or want help reporting a price increase or to request an accommodation, email rx.prices@dcbs.oregon.gov or leave a message at 503-947-7200 or 833-210-4560 (toll-free). Tell us your name, phone number, the best time to call you back, and any details that will help us provide what you need. Also, tell us if you have suggestions for improving how consumers report price increases.
