Who should use the Find a Provider search tool?

Washington Apple Health (Medicaid) clients who receive the following coverage:
  • Medical - Apple Health coverage without a managed care plan
  • Dental - only if you are on an Apple Health program that receives dental coverage
  • Visit Dentist Link or call 1-844-888-5465
  • Discounted Eyewear hardware (not service) for clients 21+ go to Free or low cost Eyewear provider list
  • Family Planning Only
Important! If you are receiving Apple Health coverage through a managed care plan (i.e. AmeriGroup, Community Health Plan of Washington, Coordinated Care, Molina, or United Health) you must contact the plan to find a medical provider.

How does this directory work?

This is a directory of providers who have registered as accepting new Apple Health clients. If a provider is no longer accepting Apple Health—but has not updated their information—they may appear in the directory.

What do I do if a provider isn’t accepting Apple Health?

If you contact a provider who is not accepting Apple Health, please ask them to remove their name from the list. We apologize for this inconvenience.
For adults - eyeglass frames and lenses are not covered by Apple Health, but if you wish to buy them, you can order them through participating optical providers at discounted prices.Find a list of participating providers.
