Warehouse Surplus Request Form

The Minnesota Department of Health (MDH) Critical Supplies Warehouse currently has a surplus of certain PPE that is no longer needed to serve the COVID-19 response.
Our warehouse surplus supply contains a wide range of products including gloves, gowns, face shields, and simple masks. This is an opportunity to receive supplies from our warehouse without being in crisis. If you are interested in receiving any of these products we have at the warehouse at no cost to you, please complete this request.
These items may not be resold, nor can they be returned to the warehouse.
Orders will be filled on a first-come, first-served basis. If you do not receive an item you've ordered, that item is no longer available. Fulfillment of requests could take up to 8 weeks. The warehouse will supply shipping for smaller orders. For larger orders we will work with your facility on pick up (preferred) or delivery options.
Note: Some items have a limit on the amount you can order. This is indicated in the survey.
