Deadlines:

Level IV referral Packets for ONLY students new to FCPS since January 2019, are due October 10, 2019.
Level IV Referral Packets for all FCPS students grades 2-7 are due to the local school January 9, 2020.

Level IV Referral Guidelines

(required) Complete the Level IV Referral form linked below
(optional) Parent/Guardian Questionnaire
(optional) Submit up to four, single-sided pages of work samples
(optional) Submit up to five, single-sided pages of awards/certificates/letters of commendation
