PLEASE READ: Thank you for taking the time to fill out this form! Your answers will help the event committee know how many veterans and military personnel to expect and any needs that must be catered to. Please note that only ONE member from your party is required to fill out this form. The celebration will take place on Veterans Day, November 11, in the Chantilly High School Auditorium at 7:00pm.
