Hemlock Woolly Adelgid Surveying
Please complete this form if you are a landowner and your property falls within the parameters of a survey for hemlock woolly adelgid. (Visit www.habitatmatters.org if you are unsure if your property meets those specifications.) Currently, the Invasive Species Network (ISN) is only able to survey within Benzie, Grand Traverse, Leelanau, and Manistee Counties.

*Please note: Completing this form does not guarantee a site visit and/or survey. ISN will be prioritizing answers and responding as personnel resources and funding allow. Any questions can be directed to Invasive Species Specialist, Zach Seguin - zseguin@habitatmatters.org. Additionally, if you have received a form from ISN asking for permission to delimit and treat, this is not the same form! Please fill the form out that was sent to you.
