2023-2024 Hard of Hearing Request
Corporation Test Coordinators (CTCs) and School Test Coordinators (STCs) must use this form to report any students who require the use of the IREAD-3 Hard of Hearing (HoH) form per their formal educational plan.
The IREAD-3 HoH form removes the requirement to respond to phonics items for students who have hearing impairments that prevent them from receiving phonics instruction. Students must still respond to additional items in Segment 1 to receive a valid scale score.
Students with the HoH accommodation indicated in the Test Information Distribution Engine (TIDE) for ILEARN Science (i.e., grade four and grade six) and ILEARN English/Language Arts will not receive items that may be sensitive to students who are deaf or hard of hearing, such as, items that require a student to write a narrative about how music makes them feel or items requesting students to draw upon experiences pertaining to sound and/or music. Students who have the hard of hearing accommodation indicated in TIDE for ILEARN Science will have items filtered that contain audio components.
This form is not required to be completed for students who have the accommodation selected on a current, finalized IEP or Service Plan. Accommodations from Indiana IEP (IIEP) are imported to the Test Information Distribution Engine (TIDE) via nightly Data Exchange.
If submitting for ILEARN Science or ILEARN English/Language Arts, this form is only required for students with the accommodation indicated on a Section 504 Plan or CSEP. Students must have the new assessment accommodation, “Student is provided a hard of hearing feature,” indicated in their current, finalized Individual Education Program (IEP). The case conference committee (CCC) must determine that the accommodation is appropriate for the student. If a student requires revisions to their IEP, parent permission is required.
