How to Get Vaccination Records

1. Download your record from the Docket app or its web version. This secure application allows you to directly review immunization history and share official vaccination reports for health, school, travel, etc.
   
   • Set up your account
   • Download the free app to your mobile device from the App Store or Google Play Store, or visit Docket's web version.
     
   • Follow the verification steps
   • Search for your immunization records
   • You must enter demographic information (name, date of birth, legal sex)
     
   • Access your records
   • Export and download PDFs of records directly to keep for records or share as you choose.

5. Request records through your health care provider. Patients can request copies of their immunization records through their health care provider. All immunization service providers in Alaska are required to utilize VacTrAK to document vaccine administration. VacTrAK does NOT provide access to patients, only to health care providers.
