Docket® is commissioned and directly endorsed by the following official government entities: Alaska Department of Health, Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, Utah Department of Health and Human Services, and Wyoming Department of Health.

Alaska, Idaho, Minnesota, New Jersey, Utah, and Wyoming residents with a valid phone number and/or email address on file with their state's immunization registry can access digital immunization records with the Docket® app.

Use of Docket® is entirely optional.

Docket Health, Inc. partners with public health to make it easier than ever for you to access personal and family immunization records. Track upcoming shots, review past immunizations, and share your official immunization reports.

I have tried so many times to get my records, and every time, I get a duplicate records message. I contacted MNDH and they have found no reason for this. I have deleted my account, uninstalled the app, reinstalled the app and created a new account at least a dozen times. I still get the same message. I've reached out to support and they can't even bother to respond. DONE WITH IT!

The ux for this app is trash, pure and simple. Example Android users are forced to use an idiotic calendar widget to set their date of birth for start you're doing a lot of swiping. Idiotic, moronic and insulting what a piece of trash. If you succed in setting the data correctly, it doesn't save the correct data. Non intuitive to say the least. NJ could have done much better. Uninstall is the best choice, they were either the lowest bidder or they have a relationship with state otherwise.

This app has a horrible user interface & design, everything it tries to have you do is confusing. Like every step. Asks questions - there is no save option (example:personal info, no save option after updating), buttons cover screen text, picking a date is overly complicated, even on screen instructions are not clear (like update year note, there are 2 yrs on widget). Think I'll just continue to keep a photo of my vaccination card on my phone instead of this app. Don't waste your time.
