The current COVID-19 global pandemic is a worrying time for us all. We are currently living in unprecedented times, and people with a Learning Disability need our support more than ever.

With government guidance changing and social distancing and shielding measures being put into place, it is important to reflect as a community and share best practice.

Over recent weeks lots of issues relating to COVID-19 and the Learning Disability population have been raised. From individuals with limited diets not being able to access the foods that they would normally eat,due to limitations in supermarkets. To the recent controversial NICE guidelines,where individuals admitted to hospital will now be assessed on admission using the clinical frailty scale. NICE have now stated: “The CFS should not be used in younger people, people with stable long term disabilities (for example,cerebral palsy), learning disabilities or autism. An individualised assessment is recommended in all cases where CFS is not appropriate.” NICE guidelines(2020). Alongside temporary changes to the Mental Health Act and adult social care, it is a worrying time for us all.

As learning disability nurses, thinking outside the box is what we do best. @WeLDNurses invite you to share your experiences of how you,and people with a learning disability are coping during this pandemic. This was the first chat that @WeLDNurses had hosted about Coronavirus, and understandably a very busy chat with a lot of important points raised and a chance to gain some insight into how other people were feeling or managing similar situations.

The main concerns which came out of the tweet chat were:

  • People assuming that the lives of people with learning disabilities are not important and they do not matter – It is important that we continue to advocate for people with a learning disability. There have been concerns raised that due to the NICE guidance people with a learning disability my be denied hospital treatment.
  • How Coronavirus is being explained to people with learning disabilities, are they receiving information? – lots of links were provided for easy read information, some of which can be found on the EBP links, also use of repetition and a lot of reassurances needed to ensure patients have an understanding of why routines may have been changed at this time.
  • There is a high risk of breakdown of care provision – the care professionals are very vulnerable now, as is the rest of the general public, self-isolation in high risk groups or symptomatic carers is affecting staff numbers which is causing great pressure of supplying a service.
  • The impact that Coronavirus may have on quality of life – the rules of lock down is affecting the lives of everyone, but a person with a learning disability may not understand why they cannot go to the shop or see their family and friends.  The loss of their daily routines will undoubtedly have an adverse effect on their well being and quality of life.
  • Diagnostic overshadowing – may mean that coronavirus is not recognised.  It’s even more important now to recognise the signs that people with learning disabilities are unwell, baseline measurements are vital.  Some mentioned more regular observations, particularly taking temperatures of patients and staff daily or more often.
  • Lack of support for people with a learning disability – services are reduced, and individuals may are unable to see family as they would normally. This could have a detrimental impact on the person with a learning disability. It was mentioned how video calling is being used more. Social stories are also used to explain the reasons why people are unable to see their family.
  • Lack of support for families – with reduced services, families may not be able to access support and there may not be access to respite services. Concerns were raised about how families will cope.
  • People with learning disabilities may not be able to maintain their independence and well-being – changes in routine could be detrimental to the person’s well-being.It was mentioned that self-isolation could lead to no routine for some.
  • Staff may struggle to support at a distance – individuals may struggle with social distancing, particularly individuals who struggle with social norms.
  • There is already evidence that the learning disability population has increased health risks and the biggest cause of death is respiratory illness
  • Lack of PBS support – changes in support may make PBS plans difficult to implement which could potentially lead to behaviours which challenge, and difficulties in being able to manage these behaviours.
  • The impact there could be if there were to be a coronavirus outbreak in a care home– Concerns were raised as to how individuals would self-isolate if they were living with others.

It was also highlighted that direct payments are becoming an issue. Where people receiving direct payments for care and the care is now no longer available these individuals are struggling to find support.

There are concerns around individuals becoming anxious and distressed which could lead to an increase in sedative medication being used. This is a concern as a lot of work has been put into STOMP, which could be undone by the pandemic.

There are concerns that not all hospitals have health liaison nurses so people with a learning disability may be treated by nurses who have limited understanding and training around supporting a person with a learning disability.

Some of the positives which were highlighted were:

  • The amount of easy read material which has been produced and shared widely in a very small space of time.
  • The use of skyping etc. which has been used by community nurses and by people with learning disabilities to stay in touch with family and friends
  • Virtual hospital passports
  • Virtual meetings
  • More streamlined ways of working
  • Sharing coping strategies
  • Greater use of “in house activities”
  • Conversations around a “good death”

Although we are living in a very difficult and uncertain time, the passion of learning disability nursing has really shone through. It is important that we continue to work together to support the lives of individuals with a learning disability in these times of uncertainty.

Being denied hospital treatment. Recieved an email with a RESPECT form today “for my information” stating “no treatment if required, palliative care in a care home”. @WeLDnurses The principle is fairly straight forward, whether people can cope with it is another matter - 7days if u symptomatic, 14days if someone in your house is symptomatic - sounds simple but is really tough for people with LD to manage #WeLDNs

@samabdulla When offered the opportunity to leave practice, they have stayed. They have chosen to help. I take my hat off to them for having the courage to do that in such uncertain circumstances. Daily or weekly check ins with ST/N’s, I hope, has helped them to feel supported. #WeLDNs
