Office of Violence Prevention Community Input 

* 1. The purpose of this survey is ask our community what forms of violence in Minneapolis should be prioritized. The results will be used to guide the Health Department's Office of Violence Prevention's first strategic plan.

You, as a part of the Minneapolis community, know best about pressing issues. Your opinion is valuable to our community. 

Your privacy is important to us: What you provide in this survey may be shared with Minneapolis Health Department employees. You are not legally required to take this survey. You will not be asked for your name, but we ask of some information about your race, age, and neighborhood. No consequences will be taken against you if you leave the survey or choose not to take the survey. 

This survey will take approximately 5-10 minutes.
