COVID-19 Vaccine Appointment Opportunities

Vaccine Appointments Available
at JCPenney Fixed POD Site

Appointments are available for ages 18 and older for Tuesday, April 20th at our JCPenney Fixed Point of Dispensing (POD) site for Moderna vaccinations.

IMPORTANT Reminders Before You Book An Appointment

You must show proof of your eligibility

Residents 18 years and older, please be prepared to show one of the following forms of ID at Dutchess County-operated sites:

  • Driver’s license or non-driver ID
  • Birth certificate issued by a state or local government
  • Current U.S passport or valid foreign passport
  • Permanent resident card
  • Certificate of Naturalization or Citizenship
  • Life insurance policy with birthdate
  • Marriage certificate with birthdate

 

You must be available for your second dose appointment

Moderna is a two-dose vaccine series. Second dose appointments will be scheduled on-site immediately following your first dose. You must be available to return to the same location approximately 28 days (on or about May 18th) following your first dose.

If you will not be available to return for your second dose, please do not schedule an appointment.
